Check your inbox for an email from HelloSign. Your completed form should be attached as a pdf. It may take a few minutes to arrive.
Click the button to find your election authority. You'll see a spreadsheet of contact information. Those counties that have verified their emails for receiving absentee ballot requests are marked verified. We hope to keep adding to the verified list. Send your completed form to your election authority.

Absentee ballot requests can be emailed, faxed, mailed, or delivered.
Mail-in ballot requests must be mailed.
Your ballot request form does not need to be notarized, but, depending on your selection, your ballot may need to be.
Missouri's law is currently being challenged in court. We'll keep this site updated. You can use the current form to request an absentee or mail-in ballot.
